I thought all totals and runlines were listed pitchers by default, no matter what. Maybe the newer US books do action on RL/totals and adjust on pitching changes?

 Bookmaker located in Costa Rica is a great book but unfortunately forces you to play action in MLB, there is no option for listed pitchers, I hope this helps you Barney.

I asked due to the MLB -1 using ML and RL -1.5. Generally you want both sides in or out. Not one side canceling if there is a pitching change. In that case you want both sides canceling.
