Question about 5dimes payout via bitcoin

I requested via cashier to payout via BTC and they sent me an email saying I need to fill out an authorization form with address, utility bill, a scan of the credit cards i used to deposit.

They said that all forms of payout require this form to be filled out, is this true?

That's not so, regrettably. 5D requires IDing each & every 6 months.
