If so, will is appear on my record or anything?

Yes anything over 10K ...including what you bet on the ticket 
( I learned that the hard way ).
No . for they're records only. ( so i was told ) .
bets over 10K , Multible ticks and casinos ( simple )
I've never signed up for one of those accounts at any Book ( f-that ) ...they might ask for it then?

So if I am lucky enough to win 15K in a Vegas sportsbook I should not collect the winnings? Because I'd have to give my SSN to collect more than 10K.

the 10k comes from the front (a money laundering enactment) and has nothing to do with taxation, however, when placing a wager of over 10k one must then present his ID & SSN to the feds by way of a form that the sportsbook is obligated by federal law to provide,(if you're not an American National you then must present your passport) still, it's the feds that get your SSN # not the book. this statue is just about impossible to enforce, you're betting 20k; you pound $9,999 twice...case closed....happens every day. with the understanding that "you don't pay taxes on money that you win by a gambling methodology" based upon the fact that it's an unrecorded income...take care

Maybe not , but it happen to me ONCE...and of course you learn how to get around that real fast.....oh and try cashing a book ticket at the Cashiers window...
