Bet Tracking Spreadsheet

Does anyone have an Excel template for tracking their bets that they would be willing to share? I'm looking for something a little more detailed than a standard win/loss/running total sheet that I can create. I'd like to be able to sort by sport among other things.

I found a few through a search on the internet but wanted to see what some of you guys use.

I don't typically track my bets outside of the local's website but wanted to start with NFL/NCAAF/NHL starting up.

I've thought many times of creating a chart/graph that identifies my wager types and amounts that alerts me visually when I'm risking too much and trending down. Number of wagers, amount of beer consumed, live betting chase....All of which could be valuable in determining flaws in behavioral management and avoiding catastrophe. 
I'd pay good money for that type of tracking.

Why there is no such program is beyond me. You can chart stock trades, trends from a million different platforms but not in gambling. Whoever can design this flow chart can make a lot of money. 
Identifying a losing streak and how to adjust would be invaluable to degenerates.
