**Most Mental illnesses are created due to CAUSE AND EFFECT. Most doctors won't tell you this though, because the amount of money spent through doctor prescriptions to pharmacies to treat these "mental illnesses" are way to high to pull the plug on.
-Most mental illnesses are caused from mis-properly treating your body and mind. Examples are; lack of sleep (#1), too many stimulants (sugar, caffeine, coke), etc.
For those flamboyant happy people that choose to change their voice, it is a cause and effect mental illness, certainly controllable, unlike down syndrome.
I certainly don't think your friend enjoyed getting beat up everyday, but once his mind made a decision that it is "OK" to become sexually aroused over the opposite sex, he went down that road. I don't know if its because of the rush of doing something bad or forbidden, but whatever it is, I do know it is a SEXUAL PERVERSION and nothing else.
My opinion on why there are so many happy people as you say almost 13%, is that most people are followers. The mind is very easy to manipulate, as the TV networks know, and very hard to get control of for the average person (liberated thinking).
Stations like MTV try to show that its okay to be happy, even going as far as video-diaries in the life of happy people. Most people watching MTV are between 10-30, the most ripe time to get at the mind.
