All of the people at the hotel were his other personalities. They only existed in his mind. The personality that was the killer was the little boy. It just so happened the one personality that survived was the little boy. So, when he escapes the van at the end of the movie he is going to keep on killing until he gets caught again.

The people at the motel existed in Malcolm Rivers' mind only. Remember, the movie kept going back and forth from the motel to those people sitting around that table (judge, lawyers, pychiatrist). Those people sitting around the table were discussing what to do about him. His psychiatrist was trying to prove that he was basically a good guy with split personalities and he should not be executed. The psychiatrist was convincing Rivers to let go of his other personalities and they began dying off (the people back at the motel). Out of all the personalities, the only one that survived was the little boy. Unfortunately, the little boy was the personality that was the murderer. That was the personality that made Rivers kill in real life, and since that personality survived, Rivers was going to keep on killing.
