The problem with health insurance is there are so many people to the point where companies just generalize in order to save money. Each persons file should be judged on its own merits.
A couple years ago I had a 0 deductible program. Everything was paid in full... from procedures-office visits and scripts. My premium started at $250. The next year it increased to $350. The next year $450 and finally it went to $600 before I switched.
