For me it was "Red Right 88"...Trailing 14-12 with less than a minute remaining in the game, the Browns had the ball on the Raiders 13-yard line and were in position for a potential game-winning field goal. Browns quarterback Brian Sipe called a time out and conferred with head coachSam Rutigliano, who called a pass play, "Red Right 88" and instructed Sipe to "throw it into Lake Erie" if the play was anything less than wide open
I would still have taken my chances with Cockroft no matter how erratic he was. This play still haunts me to this day as a lifelong Browns fan.

Too many men on the field by the Vikings in the 2009 - 2010 championship game against the Saints. Not only that, the coaches decided to throw on 3rd down instead of run to get a few yards. Favre throw the INT. Dream dead.
Ben Leber has said a million times that the defense had been wondering all year why the offense took so long to get plays in.

Schwartz last year for Det challenging that score that was already being reviewed thus making it unreviewable was pretty dumb and if I'm not mistaken Lions ended up losing the game

Woody Hayes running on the field when I was 6. 1978..in Gator Bowl vs Clemson.OhioState threw a pick and Woody punched the Clemson player for intercepting the pass.Saw it LIVE..Still the the biggest blunder ever by a coach Ive seen.Of course Woody was fired the next morning.Terrible way to see a brilliant coaching career end.He just SNAPPED...

Red Sox manager John McNamara not replacing Bill Buckner with Dave Stapleton (as he did routinely throughout the regular season) at first base for defensive purposes in the ninth inning of the sixth game of the 1986 World Series.
After the game, McNamara explained, "I wanted Bill Buckner on the field to celebrate when we won"

Thought it was awesome when Red Sox welcomed Buckner back to Fenway after they won the World Series in 2007.Felt bad for Buckner after that ball went thru..they ran him out of town then his daughter becomes a part of media.Still think Yankees-RedSox 2004 ALCS best 7game series ever seen especially when Boone sent Wakefield walking off the mound with his head down as the HR sailed over the fence the year before.

That I can give him a pass for based on respect...but fast forward 18 years later for mine
Grady Little leaving Pedro Martinez in the mother f'n game against the Yankees

I was going for the high score in Joust, and Coach Flap was in the convenience store giving me advice, as he was broke after donating so many quarters to the machine for the past few days.
And while having immense trouble killing the Shadow Lords that jump around like a crack addict, he advised me to flap my wings to the top level so they would bounce into me. However, that ate up valuable time, and pterodactyls were waiting in the wings to devour me.
Eventually, I learned to wait at the bottom of the screen for the dumb Shadow Lords to stoop to my level. But it took me another $50 of quarters before I figured this out, thanks to Coach Flap and his awful advice in a crucial situation.

Two words: Brad Childress
I was there.
Blunder for Minnesota fans.
Blessing for Saints fans.
Have you ever gone to Youtube and look at the video montages of that play and the Vikings fans reactions?

joe torre batting matt godlike kemp 6th,7th and 8th during the year where everyone in the stadium knew he was a god! mattingly already bats puig cleanup.
joe torre putting in billingsly (a starter and their best SP that season) in as a reliever to start the season.
joe torre using kuo back to back games when everyone and their mammas knew he had arm problems.
joe torre ruining the career paths and delaying matt kemp, billingsly, kuo and broxton.
there is a reason torre only won 1 championship the last decade he managed baseball.
