Been trying to get form from them for a week to complete payout process, with no luck and there not helpfull on the phone, maybe I need odds on if they payout anything???     This couldn't be good for business....

whats the issue man i use this site just got a payout the other day even though it took the full 10 business days ..

I had made deposits with disposable CC and I don't have them in my anymore, so they said I need to sign off on a form, I requested it back on 3/23. Since then they have declined payout twice..   I know there getting e-mails..., but this form is taking way to long. I guess this will be a daily follow up until they get this mysterious form out.

By disposalbe cc you mean a gift card????

yes you can call them that..., I finally got the form and sent it back... hopefully the issue will be done. I would tell you that if you do make any deposits with them, take a pic of front and back.. you will need that info. to get future payouts.
